Staff and Visitor Incident Report Forms

In the event of an accident incurred by a staff member on District property, read the Injured at Work Procedures and complete the Employee Accident Report form. The Accident Report form must be completed, and submitted to the site supervisor.

In the event a visitor or volunteer experiences an accident on District property, complete the Visitor Accident Report form (Form 697) and submit it to the building administrator/program facilitator.
